AbstractBACKGROUND: A job-exposure matrix (JEM) for inhalable aerosols, aromatic amines, and cyclohexane soluble matter (CSM) was elaborated based on measurements collected routinely between 1981 and 1996. METHODS: The data were grouped based on similarities in exposure levels and time trends in different departments, and were analyzed using smoothing splines and mixed effects models. RESULTS: Although higher than in western European countries, inhalable aerosol exposure decreased after changes in production volume and implementation of exposure reduction measures in mid-1980s. Aromatic amines concentrations first increased following the factory's production volume, but subsequently decreased in more recent years. CSM concentrations were uniformly distributed between departments. CONCLUSIONS: This JEM provides an overview of historical exposure levels in a large Polish rubber factory and will enable estimation of lifetime exposure for individual workers in a Polish rubber workers cohort and further investigation of the associations between specific exposures and cancer risk.

Due to common prevalence of occupational skin exposure and its health consequences it is necessary to look for efficient solutions allowing for reliable exposure assessment. The aim of the study is to present predictive models used to assess non-measured dermal exposure, as well as to acquaint Polish users with the principles of the selected model functioning. This paper presents examples of models to assist the employer in the the assessment of occupational exposure associated with the skin contact with chemicals, developed in European Union (EU) countries, as well as in countries outside the EU. Based on the literature data dermal exposure models EASE (Estimation and Assessment of Substance Exposure), COSHH Essentials (Control of Substances Hazardous to Health Regulations), DREAM (Dermal Exposure Assessment Method), Stoffenmanager , ECETOC TRA (European Centre for Ecotoxicology and Toxicology of Chemicals Targeted Risk Assessment), MEASE (Metal's EASE), PHED (Pesticide Handlers Exposure Database), DERM (Dermal Exposure Ranking Method) and RISKOFDERM (Risk Assessment of Occupational Dermal Exposure to Chemicals) were briefly described. Moreover the characteristics of RISKOFDERM, guidelines for its use, information on input and output data were further detailed. Problem of full work shift dermal exposure assessment is described. An example of exposure assessment using RISKOFDERM and effectiveness evaluation to date were also presented. When no measurements are available, RISKOFDERM allows dermal exposure assessment and thus can improve the risk assessment quality and effectiveness of dermal risk management. Fullerenes: Characteristics of the substance, biological effects and occupational exposure levelsSwidwinska-Gajewska, Anna; Czerczak, Slawomir; Nofer Institute of Occupational Medicine, Lodz, Poland (2016)Fullerenes are molecules composed of an even number of carbon atoms of a spherical or an ellipsoidal, closed spatial structure. The most common fullerene is the C60 molecule with a spherical structure - a truncated icosahedron, compared to a football. Fullerenes are widely used in the diagnostics and medicine, but also in the electronics and energy industry. Occupational exposure to fullerene may occur during its production. The occupational concentrations of fullerenes reached 0.12–1.2 μ/m3 for nanoparticles fraction (< 100 nm), which may evidence low exposure levels. However, fullerene mostly agglomerates into larger particles. Absorption of fullerene by oral and respiratory routes is low, and it is not absorbed by skin. After intravenous administration, fullerene accumulates mainly in the liver but also in the spleen and the kidneys. In animal experiments there was no irritation or skin sensitization caused by fullerene, and only mild irritation to the eyes. Fullerene induced transient inflammation in the lungs in inhalation studies in rodents. Oral exposure does not lead to major adverse effects. Fullerene was not mutagenic, genotoxic or carcinogenic in experimental research. However, fullerene may cause harmful effects on the mice fetus when administered intraperitoneally or intravenously. Pristine C60 fullerene is characterized by poor absorption and low toxicity, and it does not pose a risk in the occupational environment. The authors of this study are of the opinion that there is no ground for estimating the maximum allowable concentration (NDS) of pristine fullerene C60. Fullerene derivatives, due to different characteristics, require separate analysis in terms of occupational risk assessment.

(2006-01)The northeast Nile Delta region exhibits a high incidence of early-onset pancreatic cancer. It is well documented that this region has one of the highest levels of pollution in Egypt. Epidemiologic studies have suggested that cadmium, a prevalent pollutant in the northeast Nile Delta region, plays a role in the development of pancreatic cancer. OBJECTIVE: We aimed to assess serum cadmium levels as markers of exposure in pancreatic cancer patients and noncancer comparison subjects from the same region in Egypt. DESIGN AND PARTICIPANTS: We assessed serum cadmium levels of 31 newly diagnosed pancreatic cancer patients and 52 hospital comparison subjects from Mansoura, Egypt. EVALUATION/MEASUREMENTS: Serum cadmium levels were measured using a novel immunoassay procedure. RESULTS: We found a significant difference between the mean serum cadmium levels in patients versus comparison subjects (mean+/-SD, 11.1+/-7.7 ng/mL vs. 7.1+/-5.0 ng/mL, respectively; p=0.012) but not in age, sex, residence, occupation, or smoking status. The odds ratio (OR) for pancreatic cancer risk was significant for serum cadmium level [OR=1.12; 95% confidence interval (CI), 1.04-1.23; p=0.0089] and farming (OR=3.25; 95% CI, 1.03-11.64; p=0.0475) but not for age, sex, residence, or smoking status. CONCLUSIONS: The results from this pilot study suggest that pancreatic cancer in the East Nile Delta region is significantly associated with high levels of serum cadmium and farming. RELEVANCE TO CLINICAL PRACTICE/PUBLIC HEALTH: Future studies should further investigate the etiologic relationship between cadmium exposure and pancreatic carcinogenesis in cadmium-exposed populations.
